Abstract
Background: Liver disease modifies thyroid hormone metabolism, as well as a number of systemic disorders can have an effect on both the thyroid as well as the liver. Aim & objectives: To assess the thyroid derangements in cirrhotic individuals & its relationship to the severity of the liver disease. Patients and methods: This case control study included 150 cases of liver cirrhosis and 50 apparently healthy controls recruited from ward and outpatient clinic at Al-Hussin and Bab al sharia Hospitals, Department of hepatology, gastroenterology and infectious diseases between January 2023 till June 2023. Results: We found that Cirrhotic patients have higher TSH level, lower free T3 and free T4 and higher thyroid volume compared to control group. the lowest level of free triiodothyronine was among the Child C group, followed by the Child B group, the lowest level of FT4 found were in the Child C group followed by the Child B group, The highest level of thyroid-stimulating hormone was found in the Child C group followed by the Child B group. There was a statistically significant negative correlation between Free T3 and free T4 level & MELD score, between Thyroid volume by US and PC% & albumin level and between thyroid-stimulating hormone level and PC% & albumin level. Conclusion: thyroid measures have the potential to be exploited as a prognostic indicator in cirrhotic individuals. A low FT3 level can be employed in predicting which patients more prone to develop hepatic encephalopathy.
