Abstract

Introduction: The most prevalent gynecological cancer in Europe and the USA is the endometrial carcinoma (EC) and it is considered the 3rd popular gynecological malignancy in Egypt behind breast and ovarian cancer. Aim of the work: To assess recurrent vaginal bleeding, hypertension, endometrial thickness and age risk-score model’ value in predicting endometrial carcinoma in Egyptian population women with endometrial thickness more than 4 mm in symptomatic postmenopausal period. Patients and methods: We prospectively studied 200 Postmenopausal bleeding women (PMB) with vaginal bleeding and endometrial thickness>4mm undergoing diagnostic hysteroscopy, Presented to Outpatient Clinic in AL-Hussein University Hospital, during the period from September 2022 and May 2023. Result: Statistically significant increased age, BMI, post-menopausal duration and percentage of repeated bleeding from vagina in malignant group when compared with benign group were observed. In addition, significant increased parity in malignant group than benign group, while, no significant difference between two groups as regard age at menarche. Statistically significantly increased the thickness of the endometrium in malignant group when compared with benign group (median: 22 vs 10 mm). Recurrent vaginal bleeding, hypertension, endometrial thickness and age (RHEA) score can be used to differentiate between benign and malignant groups at a cutoff level of >5.5, with 84% sensitivity, 90.9% specificity, 90.2% PPV and 85% NPV (AUC=0.94 & p-value5.5 among symptomatic postmenopausal women.
