Abstract

Background: The term "undifferentiated arthritis" (UA) is a clinical term that is utilized to define a brief history of symptoms of early inflammatory arthritis, often lasting between six weeks and one year.

Aim and objectives: To estimate the role of serum Anti-Mutated Citrullinated Vimentin Antibodies (anti-MCV) level and musculoskeletal ultrasound (MSUS) as a diagnostic aid in patients presenting with undifferentiated arthritis.

Patients, and methods: This was a cross-sectional investigation conducted on the forty healthy volunteers served as a control group and fifty patients with UA as patients' group, they were selected from rheumatological and clinical immunology outpatient clinic and internal medicine department of Al Zahraa hospital.

Results: There was highly statistically significant (p-value less than 0.001) Positive correlation between anti-MCV, Synovitis score GS US, synovitis score PD US, tenosynovitis score GS US and tenosynovitis score PD US. As regards ESR association, there was a highly statistically significant (p-value less than 0.001) Positive correlation between ESR, synovitis score by GS US, synovitis score by PD US, tenosynovitis score by GS US, and tenosynovitis score by PD US.

Conclusion: Anti-MCV in undifferentiated arthritis patients may be a reliable autoantibody marker of inflammatory arthritis, which is explained by a positive correlation with inflammatory markers. Also, the positive correlation between AMCV, ESR, and CRP with US7 score reflects the role of ultrasound in the prediction of the activity of inflammatory arthritis.
