Abstract
Background: Osteoporotic patients have lower bone mineral density due to changes in bone microstructure and are more prone to fragility fractures. Osteoporotic fractures increase the risk of morbidity, disability, and death, while significantly lowering the quality of life.
Aim and objectives: To establish a correlation between serum carboxy-terminal cross-linked telopeptide of type 1 collagen (CTX-1) and bone mineral density (BMD) in persons diagnosed with type 2 diabetes mellitus (T2DM) who are undergoing pharmacological treatment for glycemic management.
Patients and methods: The case-control study gathered data from Al-Azhar University Hospitals in Cairo, Egypt, from January 2023 to June 2024, and it included 70 type 2 diabetic patients on antidiabetic medications and 30 healthy individuals. BMD and CTX-1 changes were assessed.
Results: When comparing premenopausal and male groups based on BMD percentage, the thiazolidinediones (TZDs) group showed the greatest decrease, followed by sodium-glucose co-transporter-2 (SGLT-2) inhibitors and no statistically significant BMD changes among other groups. CTX-1 levels in this study change in TZDs and SGLT-2 inhibitors groups and have a negative correlation with BMD changes.
Conclusion: Premenopausal women and men are equally affected by the effects of different antidiabetic medications on bone health. Metformin plus sulfonylureas do not negatively impact bone health. No matter your age or gender, TZDs will enhance your bone resorption. Dipeptidyl peptidase-4 Inhibitors and glucagon-like peptide-1 agonists had no detrimental effect on bone metabolism. Increased bone resorption due to SGLT-2 inhibitors is indicative of harmful effects on bone mass.
