Abstract
Background: Approximately 50% of lower extremity injuries are ankle sprains, making them one of the most prevalent orthopedic ailments. Laterally developed ankle sprains usually happen when the backfoot is inverted and rotates inward too much as the leg is outwardly rotated.
Aim of work: To evaluate the diagnostic role of ankle arthroscopy for associated intraarticular lesions in case of chronic lateral ankle instability (CLAI).
Patient and methods: Fifteen patients in Al-Azhar University Hospitals (AL-Hussein and Bab-Elshaaria University Hospitals) with symptomatic CLAI who did not improve after six months of conservative management (including male and female patients who are at least eighteen years old gave their informed consent, and were available for a minimum of six months of postoperative follow-up).
Results: According to arthroscopic findings of the series ligaments, extensive fraying, partial/complete tearing, or absence of the anterior talo-fibular ligament (ATFL) was evident in all (100% patients) 2-Synovitis (66.6 %) patients had synovitis.3- Osteochondral defect of talus:5(33.3%) patients had osteochondral defect4-Loose body:(13.3%) patients had loose bodies 5- Anterior tibial osteophytes:1(4.2%) patient had anterior tibia osteophytes. According to postoperative complications,:(13.3%) of patients suffered from mild swelling. Also, 13.3% of patients suffered from Superficial wound infection, and only (6.6%) of patients suffered from neurovascular injury.
Conclusion: Ankle Arthroscopy has a great diagnostic role in CLAI. In all (100% of the patients) with CLAI, there was evidence of partial or full tearing or disappearance of the anterior talofibular ligament. The related intraarticular lesions associated with about 90% of patients with CLAI include articular cartilage injuries, bony or soft tissue impingement, syndesmotic injuries, etc.
