Abstract

Background: Globally, gastric cancer(GC) ranks second in terms of cancer-related mortality, with Korea and Japan having the greatest rates of occurrence. More than 783,000 patients lost their lives to GC in 2018, despite a consistent drop in the disease's incidence and mortality in recent years, with an estimated 1,000,000 new cases being identified.

Aim and objectives: To evaluate the viability of laparoscopic distal gastrectomy(LDG) in cases with operable stomach cancer and to compare the intraoperative and short-term results of LDG with open distal gastrectomy.

Patients and methods: In this prospective trial, which ran from August 2023 to August 2024, 20 patients who had distal GC presented to the Al-Azhar University Hospital outpatient clinic. Every patient in the research needed to have their tumors completely removed in order to cure themselves. Patients were randomized prospectively, blindly, to receive laparoscopic or open surgery.

Results: In the pen and laparoscopic groups, the total number of lymph nodes(LN) was 23.9±4.2 and 24.1±3.9, respectively. The positive lymph node count was 5.9±1.1 in the group receiving open surgery and 6.1±0.9 in the laparoscopic group, with no statistically significant distinction among both groups(P-Value=0.2 and 0.5, respectively). All patients in both groups had a negative resected margin.

Conclusion: Comparing laparoscopic gastrectomy(LG) to open gastrectomy, the former had comparable mortality and oncologic results but a shorter postoperative hospital stay, lower postoperative morbidity, and improved postoperative quality of life. To some patients with GC, the advantages of the laparoscopic technique can be safely demonstrated by an early rate of recovery following surgery.
